Membership Registration

2018-2019 Season Membership

We are looking forward to a fun, musical ride this season.  Each chorister is asked to fill in the Registration Form below (even if you have filled one out in the past), choose a Payment Option and Payment Method. Registration Forms and payment is due no later than September 5, 2018.

First Rehearsal: Wednesday, September 5, 2018 from 6:45pm- 9:00pm at St. Stephen’s Community House (91 Bellevue Avenue).   If you are new to Common Thread, we will be holding an Orientation Session on September 5 from 6pm – 6:40pm at St. Stephen’s Community House – please join us!

Common Thread Annual Membership Fee: $400

Registration begins today. 

Due date: As soon as possible, but no later than September 5.

Payment Options:

One Payment – $400  Online available now. Cheque dated no later than September 5, 2018

Two Payments  – $200  Online available now. Cheques dated September 5, 2018 AND January 1, 2019

Nine Payments – 1 Initial Payment of $100 dated no later than September 5, 2018 AND 8 postdated cheques of $37.50 dated: October 1, 2018;  November 1, 2018;  December 1, 2018; January 1, 2019;  February 1, 2019;  March 1, 2019; April 1, 2019; May 1, 2019

Members who wish to apply for a subsidy must contact the Chorus Administrator directly at info@commonthreadchorus.ca

Payment Methods:

  1. ONLINE:  The benefits of paying online through our Canada Helps page is that the process is very easy and you get a donation receipt! (Please note that although Canada Helps calls it a donation, it really is for membership … our membership fee is a tax-deductible donation to Common Thread Chorus.)   2. CHEQUES: You may choose to pay by cheque if using the 2 or 9 payment schedule. Please mail your cheques to Common Thread Chorus, 58 Cecil Street, Toronto, Ontario, M5T 1N6 OR give all your post-dated cheques to the Administrator at the first rehearsal.

Contact and financial information is collected for internal administrative purposes and is held confidentially by the Chorus Administrator and the Treasurer. Please update your information if it should change during the year.

LEAVE OF ABSENCE: If you wish to go on a leave of absence for the next season, please indicate this by filling in a Leave of Absence form. Please inform the Chorus Administrator if you are leaving the choir without going on LOA.
